Online Registration Successful

Thanks for registering with the Dalston Practice.

Your registration is almost complete and we will call or text you to confirm your registration, or if any further steps are required. Sometimes we may ask for photo ID and proof of address to help us locate your previous NHS medical records or if you request access to online services.

You will also be offered an appointment to attend the surgery (any weekday, Monday - Friday) to have a new patient health check with the practice nurse. You can click here for directions to the practice. Please also fill in and bring with you the Health Questionnaire (or alternatively, this can be filled out when you attend). You will also be able to book an appointment to see a GP (usually within 24-48 hours depending on the urgency of your problem).
